AIDS (stage B2)
A 52 year old male complained over the previous 12 months of
dysphagia, loss of taste, and episodes of oral candidiasis which
recurred despite symptomatic antimycotic treatment (fluconazole).
Oral examination revealed white semiadherent plaques on an erythematous
base, bilaterally, on the ventral surface of the tongue (figures
1 and 2).
Microscopic examination of material taken from the lesions revealed
abundant yeast cells and pseudohyphae. Culture produced confluent
growth of a Candida albicans strain belonging to the fuzzy
phenotype. The minimum inhibitory concentrations (MICs) to azoles
were increased, with break points indicating intermediate sensitivity.
Blood tests for underlying immunodeficiency or other systemic
disease known to predispose for oral candidiasis were requested.
