ODONTOGENIC KERATOCYST

SUMMARY It is well established that odontogenic keratocyst (OK) has a propensity to recur. Numerous studies have been carried out to determine whether any particular features predispose to recurrence. The purpose of this study is to investigate the clinical features, the surgical methods of treatment in correlation with the recurrence rate of 29 keratocysts in 27 patients, 16 men (59.3%) and 11 women (40.7%). Cysts associated with the basal cell naevus syndrome have not been included. In two patients multiple presentation of the lesion was found. The age distribution at the time of operation ranged from 10 to 74 years. The surgical method used were: enucleation with primary closure, enucleation with packing, marsupialization and masrupialisation followed by enucleation in a second stage. The x2 and Fisher's exact test were used to test the statistical significance differences between the groups. The OK were observed in both jaws, 72.4% in the mandible and 27.6% in the maxilla (rate 2.6:1). Radiographically 21 cysts (72.4%) had unilocular appearance and 8 (27.6%) multilocular. The preoperative diagnosis keratocyst had been ascribed in 14 cases (48.3%). Recurrence was observed in 7 out of 29 OK (24.1%). All seven lesions that recured were located in the mandible. Multilocular OK recur significantly more often than unilocular (p=0.008). Six cases of recurrence appeared up to 3 years after the surgical treatment and one after 12 years, but the follow up in this case has not been regular. Long term follow up is necessary in order to make a diagnosis of recurrence without delay.
