SUMMARY In this study, a case of focal osteoporotic bone marrow defect (FOBMD) of the mandible of a 42 year-old woman is presented, as well as an analysis of the clinical and histopathological features of 13 additional FOBMDs of the jaws reviewed. Thirteen of the fourteen cases occurred in women who ranged in age from 24 to 78 years old and the mean age was 49.9 years. The most common location (nine cases) was the mandibular premolar/molar area. The majority of the FOBMDs presented as asymptomatic radiolucencies, while four cases were associated with pain and one of these with swelling. The clinical and radiographic appearance of the FOBMD are not pathognomonic and the final diagnosis is based on the histopathologic findings. This entity has to be included in the differential diagnosis of the radiolucent lesions of the jaws, especially for asymptomatic lesions which are located in edentulous segments of the posterior mandible.
